Abstract This study aimed to compare the effects of inspiratory muscle training (IMT) and McKenzie training on craniovertabral angle (CVA), muscle activity, and pulmonary function in individuals with forward head posture (FHP) to identify the more effective intervention. Methods: This study included 20 individuals with a CVA of 50° who were randomly assigned to either the IMT group (Group Ⅰ, n=10) or the McKenzie training group (Group Ⅱ, n=10). Both groups underwent training twice a week for 4 weeks. CVA was measured using a protractor, muscle activity was evaluated via electromyography using a 4DMT system, and pulmonary function was assessed using a spirometer. Pre- and post-intervention assessments were conducted for both groups. Paired t-tests were employed to analyze within-group differences and independent t-tests to compare between-group differences. Results: The results of this study indicated that in Group I, sternocleidomastoid muscle activity and pulmonary function significantly improved but CVA did not markedly change. Meanwhile, in Group II, significant improvements in CVA and upper trapezius muscle activity were observed, but not in pulmonary function. Conclusion: These findings suggest that IMT and McKenzie training provide partial benefits for individuals with FHP. However, McKenzie training may be more effective in improving posture-related parameters, such as CVA and upper trapezius activity, whereas IMT may be more beneficial for enhancing pulmonary function.
